By using VerifyChart you agree to these terms.

VerifyChart provides AI-assisted documentation quality review. It is not a medical device and does not constitute medical advice.

VerifyChart's analysis is adapted from the PDSQI-9 framework (Provider Documentation Summarization Quality Instrument), published in the Journal of the American Medical Informatics Association, 2025. VerifyChart analyzes notes from any medical specialty. Specialty-specific enhanced checks are currently available for Cardiology and Family Medicine.

• De-identify all notes before pasting
• Do not paste real patient PHI
• You are responsible for clinical decisions and note content
• The signing physician remains solely responsible for all documentation

Your note text is never written to our database — only scores, flag counts, and analysis results are stored. You agree not to submit protected health information. Our system detects and blocks common PHI patterns but cannot guarantee detection of all formats. Example: Unusual PHI formats or heavily abbreviated identifiers may not be detected. Always review your note manually before analyzing.

VerifyChart offers a Chrome browser extension that opens the VerifyChart web application alongside your browser. The extension embeds verifychart.ai in a sidebar panel — it does not read, access, or modify content from any webpage or EHR you have open.

The same rules apply in the extension as on the website: de-identify your note before pasting, PHI is never stored, and you remain responsible for reviewing all analysis results before signing any clinical note.

The extension reads your verifychart.ai session to keep you signed in across tabs. It does not access any other website data.

VerifyChart provides documentation quality indicators only. Results may vary between analyses. This is expected AI behavior. VerifyChart does not guarantee clinical accuracy or completeness of any analysis.

Analysis scores are computed algorithmically from identified findings. Scores are informational only and do not constitute medical advice or certification of clinical accuracy.

Re-analysis: If you edit your note and run a new analysis, each analysis is treated as a separate event. Free tier users: each re-analysis counts toward your monthly limit. Pro users: re-analyses are unlimited.

VerifyChart is not liable for clinical decisions made based on analysis results. The physician remains solely responsible for all clinical documentation.
