VerifyChart provides AI-assisted documentation quality review. It is not a medical device and does not constitute medical advice.

VerifyChart's analysis is adapted from the PDSQI-9 framework (Provider Documentation Summarization Quality Instrument), published in the Journal of the American Medical Informatics Association, 2025.

• De-identify all notes before pasting
• Do not paste real patient PHI
• You are responsible for clinical decisions and note content
• The signing physician remains solely responsible for all documentation

You agree not to submit protected health information. Our system detects and blocks common PHI patterns but cannot guarantee detection of all formats. Example: Unusual PHI formats or heavily abbreviated identifiers may not be detected. Always review your note manually before analyzing.

VerifyChart provides documentation quality indicators only. Results may vary between analyses. This is expected AI behavior. VerifyChart does not guarantee clinical accuracy or completeness of any analysis.

VerifyChart is not liable for clinical decisions made based on analysis results. The physician remains solely responsible for all clinical documentation.
